ERG11 (CYP51) mutations have been identified in some fluconazole-resistant isolates and chromosome duplication (Chr1 disomy) increases. ERG11 copy number and AFR1 transporter copy, driving heteroresistance. Specific substitutions (e.g., G484S) have been linked to clinical resistance in case reports. Resistance often involves combined mechanisms: ERG11 change, efflux, aneuploidy. Cryptococcus uses genomic plasticity (aneuploidy) as a rapid adaptation route under drug stress.
